Documentation Guidelines

Office for Students with Special Needs (OSSN)

The Rehabilitation Act of 1973 (Section 504) and the American Disabilities Act (ADA) of 1990 state that qualified students with disabilities who attend college and universities may be entitled to reasonable accommodations.  Under these laws, a disability is defined as any physical or mental impairment which substantially limits a major life activity, specifically, learning.

Although the Okaloosa-Walton College provides accommodations for eligible students in the classroom, it does not provide the testing necessary to evaluate a disability for a student.  It is the student’s responsibility to provide the appropriate documentation to the College and to request the accommodations.

The following links provide guidance to students with special needs, parents, and physicians or licensed professionals in meeting the student’s responsibility to demonstrate and/or document
the nature of the disability and the educational limitation it imposes.

NOTE:  A doctor’s prescription pad note or a school plan such as a Individualized Educational Plan (IEP) or 504 plan is NOT sufficient documentation in and of itself but can be used as part of a more comprehensive evaluative report.
